Discharge
Our hospital discharge time is 11 a.m. Whenever possible, we ask that you make arrangements to leave by this time once your
physician has discharged you. This time allows us to prepare your room for use by other patients. Our staff can assist you and
your family in making arrangements for discharge.
What to bring, a patient checklist
- Advance directive
- Photo identification, such as a drivers license, military or government issued card
- Birth certificate copy for minors
- All insurance cards
- Medicare or Medicaid cards, if applicable
- Employer information for worker's compensation claims
- Responsible party information (person responsible for the bill)
- Automobile insurance card, if your visit is the result of an automobile accident
- Personal information such as phone numbers, addresses and emergency contact information
- Your doctor's full name, address and phone number
- Pre-authorization, referrals and/or any other information regarding why you are at the hospital.
Financial Counseling
Financial counselors can help if you have concerns about how you will pay your hospital bill. They will answer questions
about your bill and discuss payment options. If you do not have insurance, they will help you find out if you qualify for
Medicaid or other assistance programs.

Helping Hand Patient Financial Assistance Program
From time to time, we all experience hardships, financial or otherwise, and can use a helping hand. Aurora is committed to
providing high quality medical services to our patients, and to making those services affordable and available to everyone.
Our Helping Hand Patient Financial Assistance Program can offer you and your family a helping hand when you need it the most.
This program is provided to patients of Aurora hospitals and clinics.
Anyone who does not have health insurance qualifies for some level of discount through this program, regardless of income.
For more information, we encourage you to speak with your receptionist or patient financial counselor.
Patient Satisfaction Survey
After you are discharged, you may receive a written patient satisfaction survey. The intent of the survey is to allow you an
opportunity to make comments or suggestions, or address any problems relating to your experience at Aurora BayCare. Your
assistance in completing the survey is greatly appreciated, as we want to continually monitor and address possible areas for
improvement.
Pastoral Care
Chaplains are available at all times to discuss your spiritual or emotional concerns. Chaplains also assist in contacting
your house of worship for sacramental needs, or a visit from your own clergy. You may request a chaplain at any time by speaking
to your nurse or calling 288-3094 to leave a message. The Interfaith Chapel, located on the first floor between the Inpatient
Reception area and the Main Lobby, is open for use at all times for prayer.
Community Resource Library
The Community Resource Library is located on the 4th floor of Aurora BayCare Medical Center just to the left of the gold
elevators. The library is open from 8 a.m. to 5 p.m. Monday through Friday to physicians, staff, patients and the general public.
The library is designed to be a center for all of your health information and decision-making needs. Our collection contains
books, videos, reference materials and medical and health journals. A full-time librarian is available to help you find just
the piece of health information you are looking for! Books in the library may be checked out for two weeks.

Interpreter Service
All patients in need of vision, hearing, and/or language interpretive services are provided such services at no cost 24
hours/day, 7 days/week. To utilize these services, please contact your nurse.
Pet Therapy Program
Aurora BayCare Medical Center's Pet Therapy program is the largest of it's kind in greater Green Bay. Created in 2001, we
continue to grow each year. We have more than 24 volunteer dogs, a cat and a miniature horse that volunteer on a regular
basis.
